Here are images of General Motors new 4.5L Duramax 4500, or Duramax LMK. A red arrow has been used to point out the unique turbocharger and exhaust port arrangement on this engine. The exhaust ports exit into the engine valley, where the turbocharger is mounted. By locating the turbocharger close to the exaust ports, thermo efficiency of the turbocharger is increased.
